We charge the following rates for our services:

*Week-day rate constitutes Monday through Friday coverage ending at 6:00pm or earlier, plus mileage.  Evening, Weekend and holiday rates per quote.  Ancillary license fee (if applicable, expires December 31, 2009) additional.  Examination unit surcharges apply for high-volume appointment blocks.  See our FAQ page for more information on our appointment and patient processing policies.
